Each individual we support will first participate in an assessment conducted by ICR’s directorial team. This meeting will guide how we structure our services around the individual’s wants, needs, and goals. Every client is unique, so we strive to determine their specific preferences and tailor our services to them. Assessments allow us to get to know our clients and how they want to lead their lives. They also help establish a strong foundation for the successful working relationship we seek to build with all our clients, as well as anyone else they wish to be involved with the process of planning for their futures.
Each of our clients has a service plan in place which is specific to their needs and desires. This service plan is clearly outlined on a document called an Individual Support Plan, or ISP. This document is prepared collaboratively by our team and the client, and serves as a training tool for the client’s attendants. It is updated annually to reflect changes, including any progress the client has made on their goals throughout the year, and any new intentions they want to set for their future.
Each ICR client can receive assistance with the coordination of everything from medication management to medical appointments, transportation for and assistance with community activities, grocery shopping, meal planning and prep, companionship and advocacy, management of IHSS hours, maintenance of important resources such as Section 8, SSI/SSDI benefits, Medi-Cal, CalFresh, etc., participation in a Day or Work Program, and more. ICR can also assist our clients with financial management, if desired, as well as with finding and maintaining appropriate housing.

Here’s how our Referral Process works…
Individuals are referred to Supported Living services by their local Regional Center. ICR is vendored to provide services through the Regional Center of the East Bay (rceb.org) and Golden Gate Regional Center (ggrc.org). If you are not already a Regional Center client, please reach out to your local Regional Center to find out if you are eligible. If you are already a Regional Center client, you can ask your Case Manager for a referral to our agency.
